JOB SUMMARY:

The Case Aide is responsible for staff support in the Division of Developmental Disabilities     (DDD) Support Coordination Units assisting with casework directed by the Unit Supervisors, Human Service Specialists; scheduling activities for the work unit; providing accurate and professional responses to inquiries from the public and agency staff and completing brief reports as necessary.

JOB DUTIES: 

-          Conducts residential and day program visits at the direction of Support Coordinators/Supervisors for the purpose of observing for potential health and safety issues; prepares progress notes on visits; and provides information to requested party

-          Assists members/families by providing transportation to various appointments and meetings; picks up medications, bus passes, food boxes; completes forms, conducts research of community resources and completes applications for services and reports; pays bills; refers families to community resources; writes progress notes accordingly in member files

-          Prepares a wide variety of member documents, (e.g., service authorizations, correspondence, memos, forms and legal documents) for signature of designated authority for work unit and/or office. Responds to requests from various sources to update the prepared information.

-          Collects necessary information and prepares eligibility and member re-determination packets. Tracks eligibility decisions. Inputs member information into member

KNOWLEDGE, SKILLS AND ABILITIES (KSAs):

Knowledge of:

-          Basic human needs, human growth, personality and behavior

-          Basic principles and practices of case management

-          Community resources

-          Microsoft Word, Outlook, Excel

-          Tracking systems

-          General office procedures  

Skill in:

-          Applying federal laws, state statues, and agency policies and procedures

-          Recognizing basic human rights and the care and assistance needs of individuals with developmental disabilities

-          Oral and written

-          Communication

-          Basic clerical skills including proper telephone etiquette, opening office equipment (e.g., copier, fax).

Ability to:

-          Adapt and be sensitive to a variety of cultures

-          Maintain and display professional demeanor

-          Establish family and professional partnerships

SELECTIVE PREFERENCE(S):

-          Candidate must possess at least one (1) year of customer service experience or working with or assisting individuals in a community setting, (e.g. community involvement with individuals with developmental disabilities, aging and adult, youth, etc.)

-          Travel is required within the District.

PRE-EMPLOYMENT REQUIREMENTS: 

-          Level one Fingerprint Clearance Card

-          Subject to a search of both the Child Protective Services Central Registry and Adult Protective Services

-          Valid Driver’s License

-          Background and Reference Checks
