JOB SUMMARY: The Division of Developmental Disabilities (DDD) is seeking an experienced and highly motivated individual to join our team as a Bilingual Case Manager 2. The Bilingual Case Manager 2 serves individuals with developmental disabilities by supporting them and their families to lead a healthy, self-sufficient, productive life within their capabilities. |
JOB DUTIES: - Document individual progress, concerns and complete appropriate referrals to community agencies - Research community for available resources to meet needs identified in ISP and to inform individual of resource options. - Complete referrals for assessed/covered services - Develop, review, monitor and evaluate Individual Service Plan (ISP) in accordance with mandated timeliness - Collaborate with community agencies to provide assistance and support to individuals and families in crisis. - Attend meetings with individuals, families, service providers, and others as required - Write comprehensive progress notes and maintain case records - Compile information from a variety of sources in order to make recommendations for eligibility determination and re-determinations of eligibility - Confer with supervisor, attend staff meetings and training as required |
KNOWLEDGE, SKILLS AND ABILITIES (KSAs): Knowledge of: - Methods, procedures and techniques used in the development of Individual Support Plans - Federal and State statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities - Principles and practices of case management and community resources - Medical and behavioral health issues and terminology - Principles of applied behavioral interventions - Child development principles Skill in: - Oral and written communication; - Developing, evaluating and maintaining client service plans - Interviewing, counseling and interacting with clients and family members; Conducting needs assessments: - Problem identification and resolution; - Interpersonal relations as applied to contacts with clients and representatives of other governmental and community organizations and the public Ability to: - Work independently and within diverse work teams - Organize and manage case load - Document case files and information within required/mandated time lines - Maintain confidentiality - Drive a motor vehicle |
SELECTIVE PREFERENCE(S): - A Bachelor's degree or higher in Social Science, Health Education, Education (i.e. psychology, social work, health sciences, early childhood education) degree or other closely related field from an accredited college or university; OR two (2) years’ experience in a permanent/limited status position as a Human Service Specialist I: OR a combination of education and experience which must include one (1) year of social work at the professional level. - Bilingual in Spanish/English (reading and writing) |
PRE-EMPLOYMENT REQUIREMENTS: - Valid Arizona Driver's License. In-state travel is required. Requires possession of and ability to retain a current, valid state-issue driver's license appropriate to the assignment. Employees who drive on state business are subject to driver's license record checks, must maintain acceptable driving records and must complete any required driver training (see Arizona Administrative Code R2-10-207. 12.). - Employees may be required to use their own transportation as well as maintaining valid motor vehicle insurance and current Arizona vehicle registration; however, mileage will be reimbursed. - Level 1 Fingerprint Clearance Card. |
