JOB SUMMARY:

The Division of Developmental Disabilities’ (DDD) Case Manager 2 serves members with developmental disabilities, e.g., Autism, Cerebral Palsy, Intellectual Disability (for example, Down Syndrome), Epilepsy, etc., by supporting them and their families to lead the healthiest, most self-sufficient, productive lives within their capabilities.

JOB DUTIES: 

-          Managing an average caseload of 40 members and meeting with supervisor regarding case status

-          Providing support coordination that consist of identifying needs and available services and resources, planning, obtaining, and monitoring the effectiveness of contracted medical and non-medical services in meeting our members’ identified needs

-          Working with members, family members, and care professionals as a team to review the needs and develop an Individual Service Plan (ISP).

-          Meeting regularly to assess needs and supports, monitor progress, or make changes to the ISP, which includes travelling at different times throughout the day within and outside business hours to conduct onsite-visits to member’s home, placement, schools or program sites; this activity represents 30% of work time and may require driving your own personal car (Mileage will be reimbursed) 

KNOWLEDGE, SKILLS AND ABILITIES (KSAs):

Knowledge of:

-          Methods, procedures, and techniques used in the development of Individual Support Plans (ISP)

-          Health Insurance Portability and Accountability Act (HIPAA)

Skill in:

-          Oral and written communication

-          Interpersonal relations as applied to contacts with members and representatives of other governmental and community organizations and the public

-          The use of various software (Microsoft Word, Excel, etc.)

Ability to:

-          Work independently and within diverse work teams

-          Absorb, interpret, and assimilate large quantities of written material

-          Drive a motor vehicle

SELECTIVE PREFERENCE(S):

-          The candidate must possess two (2) years of professional experience coordinating social service activities (e.g., ISPs, client referral for services, community services, etc.); OR one (1) year as a Human Service Specialist I in Arizona State Service; OR a Bachelor’s or Master’s degree from an accredited college or university

-          Bilingual (English and Spanish) is preferred

PRE-EMPLOYMENT REQUIREMENTS: 

-          Have a current, valid state issued driver’s license appropriate to the assignment. Employee is subject to driver’s license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration, and must complete any required training (see Arizona Administrative Code R2-10-207.12)

-          Employee shall have a valid level one fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults

-          Candidates for this position shall be subject to a search of both the Child Protective Services Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459

-          Successfully pass background and reference checks. Employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions
