The Division of Developmental Disabilities (DDD) is seeking for a highly motivated individual to join our team as a Case Manager Lead-Early Intervention. This position is responsible for serving children with developmental disabilities and their families. Major duties and responsibilities include but are not limited to:

-  Conducting home visits and meeting with individuals/families to develop relationships and become familiar with family dynamics/priorities/resources/concerns/supports and make appropriate referrals to community agencies

- Convening and facilitating multi-disciplinary Individualized Family Service Plan meetings (a minimum of every 6 months on every child) to support children with developmental disabilities and their families

- Assisting families in choosing appropriate providers

- Assisting the families with orientation into all systems, any problem solving, & system issues which may arise

- Gathering all necessary information and documentation for determining eligibility for DDD Early Intervention services

- Developing and maintaining files for each child and writing comprehensive progress notes




- Methods, procedures, and techniques used in the development of Individualized Family Service Plans

- Federal and State statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities

- Health Insurance Portability and Accountability Act (HIPAA)

- Principles and practices of case management and community resources

- Child development principles

- Effective verbal and written communication skills

- Interviewing, counseling, and interacting with members and family members

- A Bachelor's degree in Early Childhood, Early Childhood Special Education, Family Studies, Speech-Language Pathology, Physical Therapy or Occupational Therapy from an accredited college or university; OR a Bachelor's degree in a closely related field from an accredited college or university with review of coursework and approved by the Arizona Early Intervention Program (AzEIP) and one (1) year of experience working with people with developmental disabilities; OR Developmental Specialists or Service Coordinators (Support Coordinators), hired in the AzEIP system before July 1, 2001, documentation of continued satisfactory work in the AzEIP system from that date of hire, and three years  of Service Coordinator (Support Coordinator) experience which must include one (1) year of  experience working with people with developmental disabilities, are required

- Bi-lingual (English/Spanish) is desired
