JOB SUMMARY:

The Division of Developmental Disabilities’ (DDD) Case Manager Lead is responsible for serving persons with developmental disabilities by supporting them and their families to lead the healthiest, most self-sufficient and productive lives within their capabilities.  

JOB DUTIES:

- Conducts on-site visits to members’/families’ homes and meets with members/families to develop and become familiar with family dynamics/needs/supports and makes appropriate referrals to community agencies; convenes and facilitates multi-disciplinary Individual Support Plan (ISP) meetings to determine and identify services 

- Researches community for available resources to meet needs identified in plan; facilitates meetings and program visits to enable member/family to choose appropriate providers; writes referrals for chosen providers to ensure that funding is secured for services

- Reviews, monitors and evaluates ISP in accordance with mandated timelines

- Makes on-site visits to member/family homes, schools and program sites to discuss member’s progress, needs, ISP, etc.

- Provides assistance and support to members/families in crisis and attends meetings with members/families, service providers and others

 

KNOWLEDGE, SKILLS AND ABILITIES (KSAs):

Knowledge of:

- Methods, procedures and techniques used in the development of Individual Support Plans 

- Federal and State statutes and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities

- Principles and practices of case management

- Community resources, medical and behavioral health issues and terminology and principals of applied behavioral interventions

- Variety of mental health diagnosis, SMI designation, medications and their potential side effects

- Conflict resolution techniques

 

Skill in:

- Oral and written communication 

- Initiating, developing and maintaining member service plans

- Interviewing Techniques

- Interpersonal relations as applied to contacts with members and representatives of other governmental and community organizations and the public

 

Ability to:

- Teach, coach, train to analyze issue and develop positive solutions

- Maintain confidentiality 

- Work independently and within diverse work teams

- Lead a team of Support Coordinators in the absence of the supervisor

SELECTIVE PREFERENCE(S):

- Candidate must possess Bachelor's Degree in Social Science, Health Education, Education, Psychology, Social Work, Health Sciences, Early Childhood Education or closely related field and one (1) year experience working with people with developmental disabilities at the professional level; OR one (1) year as a Human Service Specialist 2; OR three (3) years of case management/social work experience at the professional level including one (1) year experience working with

people with developmental disabilities

- Experience working in the behavioral health field

- Spanish bi-lingual preferred

 

PRE-EMPLOYMENT REQUIREMENTS: 

-          Have a current, valid state issued driver’s license appropriate to the assignment. Employee is subject to driver’s license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration, and must complete any required training (see Arizona Administrative Code R2-10-207.12)

-          Successfully complete the Electronic Employment Eligibility Verification Program (E-Verify), applicable to all newly hired State employees

-          Successfully pass background and reference checks. Employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions

-          Employee shall have a valid level one fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults

-          Candidates for this position shall be subject to a search of both the Child Protective Services Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459
