JOB SUMMARY:

The Division of Developmental Disabilities’ (DDD) Habilitation Nurse 2 is responsible for providing specialized nursing care for individuals with developmental disabilities requiring regular nursing care.

JOB DUTIES: 

-          Assess health needs and completes a nursing assessment of members with developmental disabilities; makes nursing diagnosis and submits to designated departments

-          Participates in fact findings related to medical issues; completes monitoring of medical care; monitors medical administration systems; provides feedback

-          Completes utilization review on caseload and documents on-going medical needs; submits to designated department; discharge and admission planning

-          Notes orders and prepares reports for basic care, nursing assessments, and Individual Service Plans (ISPs)

-          Examines and analyzes medical records and charts to ensure quality care

-          Participates in development of policies and procedures

-          Assists with problem solving related to medical policies and procedures

-          Develops and provides education classes for staff

KNOWLEDGE, SKILLS AND ABILITIES (KSAs):

Knowledge of:

-          Development Disability member nursing theory, practice, and process

-          Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/ID) and State Licensing requirements

-          State Nurse Practice Act

-          Title XIX Federal Regulations and Licensing standards

Skill in:

-          Nursing assessment

-          Communicating clearly and concisely in writing and verbally

Ability to:

-          Make independent decisions regarding member health concerns

-          Drive a vehicle

SELECTIVE PREFERENCE(S):

-          The ideal candidate will possess at least one (1) year experience in providing professional hands-on nursing services to individuals with developmental disabilities and at least one (1) year of experience in professional community nursing

-          The candidate is required at the time of appointment to have a possession of current license or temporary permit to practice as a registered nurse in the State of Arizona; within six (6) months from date of hire, the candidate must possess of a current license to practice as a registered nurse in the State of Arizona

PRE-EMPLOYMENT REQUIREMENTS: 

-          Background and Reference Checks

-          Valid Driver’s License

-          Level One Fingerprint Clearance Card

-          Subject to a search of both the Child Protective Services Central Registry and Adult Protective Services

-          Medical/Physical Test

-          Valid Professional License
