JOB SUMMARY:

The Division of Developmental Disabilities’ (DDD) Habilitation Supervisor 2 is responsible for supervising and providing leadership, mentoring, coaching, monitoring and on-the-job training to supervisors and staff who provide direct care in a 24-hour residential Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID) and State Operated Group Homes.

JOB DUTIES: 

-          Supervises direct care staff and their supervisors

-          Completes performance evaluation (MAP)

-          Participates in the development of the Individual Support Plan (ISP), ISP reconvenes, Behavior Management reviews, Psychiatric clinics, Home Risk meetings, etc.

-          Completes review of Member Incident Reports ensuring accuracy and completeness and conducts fact-finding when necessary

-          Ensures proper maintenance of premises and upkeep and cleanliness of the home

KNOWLEDGE, SKILLS AND ABILITIES (KSAs):

Knowledge of:

-          Individual Support Plan (ISP) process

-          Specialized needs for individuals with developmental disabilities

-          Supervisory principles

Skill in:

-          Verbal and communication, listening and problem solving

-          Supervisory techniques

-          Teamwork and team building

Ability to:

-          Evaluate staff performance and resolve problems

-          Monitor the provision of care to individuals, identify problems and implement solutions

-          Read and write in English

-          Drive a vehicle

SELECTIVE PREFERENCE(S):

-          Must be at least 18 years of age

-          Must be willing to work weekends, holidays, nights and overtime

-          Must have two (2) years experience supervising direct support staff in a 24-hour residential setting and at least one (1) year experience working with individuals with developmental disabilities

-          Candidates with a High School or General Education Development (GED) diploma is preferred

PRE-EMPLOYMENT REQUIREMENTS: 

-          Background and Reference Checks

-          Valid Driver’s License

-          Level One Fingerprint Clearance Card

-          Subject to a search of both the Child Protective Services Central Registry and Adult Protective Services

-          Medical/Physical Test
