JOB SUMMARY:

This position will serve individuals and families with developmental disabilities and assist in identifying and developing community resources to preserve the person’s well-being in the home and community.  Will be responsible to ensure consumers are eligible for the Division of Developmental Disabilities services following all relevant state, federal mandates, agency policy and procedures. 

JOB DUTIES: 

-          Conduct on-site visits to individuals, families, program sites to develop relationships; discuss service needs and supports.

-          Develop an Individual Service Plan (ISP).

-          Document individual progress and concerns and complete appropriate referrals to community agencies 

-          Research community for available resources to meet needs identified in ISP and to inform individual of resource options. 

-          Facilitate meetings and program visits to assist with individual, families with choices. 

-          Complete referrals for assessed, covered services.

-          Develop, review monitor and evaluate Individual Service Plan (ISP) in accordance with mandated timeliness. 

-          Approve type and level of assessed services within approved guidelines and release client funds.

-          Collaborate with community agencies to provide assistance and support to individuals and families in crisis. 

-          Attend meetings with Individuals, families, service providers, and others as required.  Identify and resolve issues, conflicts. 

-          Compile information from a variety of sources in order to make recommendations for eligibility determination and re-determination of eligibility.  

-          Write comprehensive progress notes and maintain case records. 

-          Prepare a variety of reports and complete special projects, as required.

-          Statewide travel may be required.

 

KNOWLEDGE, SKILLS AND ABILITIES (KSAs):

Knowledge of:

-          Methods, procedures and techniques used in the development of Individual Support Plans (ISP).

-          Federal and State statues and agency policies and procedures relating to the care and assistance for individuals with developmental disabilities.

-          Practices of case management.

-          Community resources.

-          Medical and behavioral health issues and terminology.

-          Principles of applied behavioral interventions. 

-          Child Development principles.

Skill in:

-          Oral and written communication

-          Developing, evaluating and maintaining client service plans.

-          Interviewing, counseling and interacting with clients and family members.

-          Conducting needs assessments,

-          Problem identification and resolution.

-          Interpersonal relations as applied to contacts with clients and representatives of other governmental and community organizations and the public.

-          Conflict resolution.

Ability to:

-          Work independently and within diverse work teams. 

-          Organize and manage case load.

-          Document case files and information within required/mandated timelines.

-          Drive a motor vehicle.

SELECTIVE PREFERENCE(S):

-          The candidate must possess two (2) years of professional experience coordinating social service activities (e.g., ISPs, client referral for services, community services, etc.); OR one (1) year as a Human Service Specialist I in Arizona State Service; OR a Bachelor’s or Master’s degree from an accredited college or university

-          Bilingual (English and Spanish) is preferred

PRE-EMPLOYMENT REQUIREMENTS: 

-          Have a current, valid state issued driver’s license appropriate to the assignment. Employee is subject to driver’s license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration, and must complete any required training (see Arizona Administrative Code R2-10-207.12)

-          Successfully pass background and reference checks. Employment is contingent upon completion of the above-mentioned process and the agency’s ability to reasonably accommodate any restrictions

-          Employee shall have a valid level one fingerprint clearance card issued pursuant to Arizona Revised Statute 41-2758.07 in order to work with children and vulnerable adults
